I found the issue with the Venomlord's chest. It was swapped with the one from the Mindshear Acolyte raid. Normally that bug wouldn't be noticed in the Mindshear raid, but the accidental swap caused the bug to show up.

I'll fix the bug and swap the chests back to where they belong.
