We've found the cause of the problem -- and yes, your information in this thread did help us immensely -- but unfortunately any solution isn't going to be able to be hotfixed. We'll have more information about what and when asap.

What we do know is that it's character specific, not the zone or the method for memorizing it. If you're experiencing this issue further attempts to memorize the tome won't help.
